Medical exam may be requested by cic . It's not mandatory. It depends on many factors - the job, location of work, travel history and most importantly, officers decision.
If one is requested for medical test for extension of work permit, cic will let you know at the time of extension.
Now You can travel to Canada without any issues.

If after going through above link you think that a Medical Request was applicable to your case but CIC did not ask for this then to be on safer side you may want to approach CIC with your query ?
But as per my experience no one will ask you at the port of entry whether you attended medical or not because as per my understanding CIC issues you a visa only after having your medical results if the medical was required in that visa category. I am not sure what will happen if you company asks you to extend your work permit then in that case CIC might advise you to go for a medical exam at that point of time when you file for your extension and then decide your extension based upon those medical results? But its just my assumption as I am not clear about your situation and your visa category/ duration etc...
