Hello everyone,

I have few questions regarding the declaration and consent forms and WES report.,

1. Does both PA and spouse needs to sign both the declaration and consent forms
2. In the Declaration of commitment form, there is no room for spouse as it says, I................, the PA How could a spouse sign this form
3. Do we need to send copies of Degree certificate apart from ordering an electronic delivery of WES report to NBPNP
