Please see my answers in BLUE

1) Is it form# IMM 5444 E, when you say complete the PR application form.
- I dont remember the number, there is only one form on CIC website for PR card application.

2)If yes, then , which option r u talking about when u say "do not tick the legal name change as you are not legally changing your name". can you please specify the option number for me, if this is the right form.

- I dont remember which option, either the main application form or the checklist that you need to submit has this.

3) can you specify which other documents do i need?

- I have mentioned all that I submitted.

4) Do u have to wait for amended PR card in order to get name rectified on SIN, Health Card or any other provincial Id?

- Ofcourse! Without that Service Canada / Service Ontario will not do it

You wont be able do either.

As a new immigrant, you will receive your 1st card on the address provided by you to the landing officer. You can start all the process only after you receive your 1st card as BLS will ask you for the original PR card.

- apply for a new passport with correct name.

Hi,
I had the same problem. My passport had only given name, because of which my COPR and PR card had captured everything under last name.

I have got everything rectified now, without getting any affidavit or legal name change procedure or name change request through any provincial offices.


1 - Submit a letter to BLS (along with passport application form to get a new passport with first name and last name) explaining the error and why you could not get it rectified earlier. (In my case the PR application was under process). - Got my new passport in 20 days

2 - Once you receive the updated passport - Submit similar letter to CIC explaining the error, along with other documents - Application form (Do not tick legal name change as you are not legally changing your name) + Cancelled page of old passport showing the new passport number and copy of the new passport showing old passport number, ensure you mention this also in the letter) + copy of the letter you submitted to BLS + any other documents. - Got my New PR card in 72 Days, Please note COPR will not change, only PR card will issued with a new name.

3 - Got my SIN, Driver Licence and Health Card updated.

Its an easy process if your letter is crisp and explains things clearly. Let me know if you have any doubts.

Update (Correct) your Name via Health Card & Drivers license.

PR Card name is not big deal, however when you apply Citizenship apply with how exactly it appears Drivers license.

Also, change of name in the driver's licence can only be done through Service Ontario and they will change it after seeing legal name change document and not without this.

Please guide on the entire process if you have done it before.


My issue is that my name in my given name passport was 'Kanwarjit Singh' last name field was blank. Now my Canadian name is Family Name 'Kanwarjit SIngh' and Given name is blank.

I had the same issue. Check how to get the legal name change in Ontario. In Manitoba you had to visit Statistics Canada to get it done. It takes around 1.5 months for the complete process to complete. Guess this this link for Ontario: -

Once you have that document, use it change your name in your DL, health card, SIN, bank accounts etc.

For PR Card, its not compulsory to get it changed. You can directly get it changed during your Citizenship application. I did the same. I am now waiting for Oath.

Also, I updated my home country passport with the split name, but it holds no value in Canada once you are PR :). So even if you change your name on Passport you can travel with your PR Card with the old name. I have traveled so can confirm, there won't be any issue.

Hi KSingh!

That is the process that CIC asked me to follow. I did that after 2 years of receiving my PR card.
If you are only splitting you name, then you need to get it corrected on your Indian passport first, then write to CIC using PR renewal form.
Once you receive your new PR card with corrected name, visit service Ontario to update your DL & OHIP and Service Canada for SIN

If you reside in Ontario, you can split your given name by applying for a name change certificate.

Once you obtain the name change certificate, you can then change the name in bank account, driving license, health card by visiting any Service Ontario centre.

You can then change your name in SIN by visiting any Service Canada centre.

Then you can change the name in your PR card by submitting a renewal application.

Simultaneously, assuming you are an Indian passport holder, you can split your name in your passport too.

If you submit the Ontario name change application now, you can expect to receive it in 6-8 weeks. It is reasonable to expect your name change certificate by end of February.

The whole process of name change including everything is around 5-6 months. Excluding PR renewal, the whole process including changing the passport might take around 4 months. There is a good chance that you might be ready to apply for PR renewal by name change by March.

If you are renewing the PR only to update the name change, then you don't need to send in the current PR card. A photocopy will be sufficient.
