Hey there,
I have a question reguarding my name on these several card. On my PR I have my name before marriage, I am still without OHIP because they wanted address proof , other than the proof we had, which by the way are on the list, but the lady there was soooo rude, so now that the driving centre in Ontario are off strike, next week I am going to apply for my G1, and later on take it to OHIP as proof of residency in Windsor. Here is the question, on my driving license I am going to ask them to use my name before marriage, so it will be identical with my PR and my secured credit card, so OHIP will except them as proof of residency. How can I later on change my name on my PR, OHIP, SIN and Driving License and in wich one I have to start first?

You have to contact the issuing agency to get your name updated, i.e.:

For the PR card, CIC.
For the OHIP card, OHIP (Service Ontario).
For the SIN card, Service Canada.
For the driver's licence, the MTO.

You don't need to start with any single one in particular. Your marriage certificate is the document you need to do the name change. I'd just do them all at once.

I don't think it really matters, I would start from SIN first as it's the fastest. In driving licence they say you need to apply for name change within 5-6 days, so it could be 2nd, then PR while OHIP is on the way.

Thank you for your reply, you mean that I don't need to start with my PR after getting my driving license since my PR is my main ID...that is good.

If I am not mistaken, you can keep your current PR card if it is still valid and you will not have to return it until the new PR card is ready. At least that is the case with renewals.

Suin, what you mean by I need to apply for name change, is it like a form I have to fill it, or by showing them my marriage certificate it will be done.
Karlshammar , you are right, I can keep my PR untill the new one is issued, it take like 1 to 2 month...at least that is what my lawyer said
