Name issue when filling application form (Section 3A, 3B)

A different Service Ontario (SO) location changed my wife's name in her DL and OHIP without going through the legal name change. ;D She will now go for SIN change. 3 different SO locations and all 3 had different response. Where in the world are we living?

My wife could not assume last name in her SIN even though it was updated in her DL and OHIP. The SIN office mentioned that she needs to get it updated in her PR Card first and then they change it. However, the funny thing is that based on her DL update, the SIN office added both first name and last name in her last name.
SIN and PR Card system should be applicable Nationwide which means it should be following same rule. However, as per you statement, you could change the SIN first and then PR Card.
Any hints on my problem and how to proceed further?

See, my scenario was different. I went through legal name change and that's why I had no problem changing my identity documents. Once all of my documents were changed, i got my wife's last name update to my surname. She had her maiden surname before. In your wife's case, its different scenario.

I would suggest you to get her name changed on PR first as SIN office only looks at the COPR or PR card as the identity document and your wife's COPR would have the old name only.
Also talk to CIC call centre too at 1-888-242-2100. Once PR card has been changed, go to SIN (Service Canada) office and get the name updated.
