Hi

My passport does not have my last name on it though I have last name as kaushal. My entire name ritu kaushal is in the column firstname and lastname is blank. That was a mistake and i did not changed it on my passport. My daughters birth certificate mentions my last name as kaushal and first name as ritu. Can cic reject my case on this ?
Please guide me. What can I do to avoid rejection ?
I have to send documents by 5th dec 2014.

birth certificate affidavit should be given - notary public can do this

would that mean I have to update my cic forms and documents that I am known by another name ? One as per passport and second as per my daughters birth certificate ?

Please guide.
