---- that's okay.....in the forms......you are required to indicate your full family name (surname or last name) as it appears on your passport, travel or identity document.

Birth Certificate has her old last name.
------ that's okay.....the birth certificate should have her father's name.
------ will advice that you check the country specific requirements if any additional required documents.

nikah nama in urdu has her old last name but the english has her new one.
----- if the nikah nama in urdu document was translated.......shouldn't the name be same in both.

Just checked. Her ID, Passport only has her new last name and no fathers name. Her birth certificate and Marriage certificate from NADRA also has her new last name. We don't have a Urdu nikahnama only english from union council and it is registered and stamped with her fathers last name. Perhaps I should write her new last name in it

I have a question, I am sponsoring my husband, outland. I haven't changed my last name to his on any of my identification documents. Will this be a problem when applying.
No.....its not mandatory the sponsor change their last name.......however, some spouses do change last name even after submitting the application just to support proof of marriage documentation.

shouldn't your spouse have the father's name as last name in marriage document......the name change is after marriage.

Also check country specific requirement.
We changed it everywhere except on the original english nikkah nama that has her fathers name other then that, its all under her new last name including NADRA marriage certificate, union council nikkah nama computerized,ID Card, Passport, Birth certificate.

Her documents can be a mix of her maiden and married name, as long as the application is in her passport name.
