Name change document

Dear sir, I have a question regarding the my ITA application.
My first name is Meenakshi and last name is Ganesan. This is how it is in my passport and so the Express Entry and ITA application.
1.My education documents has Meenakshi. G
2. My marriage certificate has Meenakshi. G
3. But most of my employment record has Meenakshi Yuvaraj, as yuvaraj is my husband's name. Hence while filling ITA, marked Yes for other names used and mentioned Meenakshi Yuvaraj.
In order to upload document for Name change, I went to notary public and got an affidavit mentioning that Meenakshi. G, Meenakshi Ganesan, Meenakshi Yuvaraj are one and the same person.
Is this document enough to upload while submitting ITA?

Any name change post marriage is proved by marriage certificate. Any other discrepancy in your documents can be taken care by same name affidavit.

Affidavits do not have an expiry date. Ideally they should be as close to your application filing date, or close to who you submit them. Affidavits are just documents signed under oath.
