Name change as child. Do I need to declare it?

Hi,

I am submitting an application for PR through federal express entry. My surname was changed by my parents when I was a young child (5-6 years old). I have never used the previous name before in any official setting or on any official document. Should I include this previous name in my application? If so, is it enough to include a letter or a sworn affidavit to prove that this was my previous name? I do not have any official documents with that name because I was a young child when it was changed.

Definitely. They will see it and it will delay your processing. I am actually dealing with this issue right now for my kid. I have applied for several permits and was never asked for any documentation. Now on pr app, they are asking for documents regarding that so it would be best to just provide them with all of the information.

Thank you! Do you know what documents I will have to produce? It will be hard for me to get a birth certificate. Is there a chance a sworn affadivt will be enough?
