This was the email I received from MPNP:

my sister’s name recent online application to the Manitoba Provincial Nominee Program (MPNP) received on 1/10/2012.
The applicant has identified you as his/her Manitoba close relative for the purposes of the application to the MPNP and has indicated you will complete a form called Settlement Plan - Part 2.
In Settlement Plan - Part 2 you are asked to describe how you will help the applicant achieve his or her plan to settle and find employment in Manitoba.
Instructions:
If the applicant has not yet e-mailed you a copy of his/her completed Settlement Plan - Part 1, request that they do so. You will need to review the applicant’s plan before you complete Part 2.

Print and complete Settlement Plan - Part 2, then submit it to the MPNP within 30 days of receipt of this message.
You must include the following supporting documents:
• proof of your Canadian citizenship or permanent resident status (copy of your citizenship card, permanent resident card or Canadian passport)
• proof of Manitoba residence for at least one year (copies of both sides of your Manitoba Health Card)
• proof of relationship to the applicant (birth and marriage certificates demonstrating close relationship)
We prefer that you scan your completed and signed Settlement Plan - Part 2 and your supporting documents and send them as attachments in an e-mail to mpnpsupporter@gov.mb.ca.
Or, you can drop off or mail your printed copies to the MPNP office in Winnipeg. You will find the address on the last page of Settlement Plan - Part 2.
