I believe it is registered mail, and technically you have received it on Jan 18, 2011, so the 30 days start on Jan 18, 2010. However, in my opinion, if you have the time and just to be on the safe side, start your counting on Jan 5. Hope you do well with your medical. If you have any previous operation, just make it sure to bring the documents about it, so they will not ask you to come back.
I had my medical in saint lukes, It was quick. Although someone said that they are more strict there, but it was just a breeze for me. just 2hrs or less. And I heard that they send records to CEM quicker, I am not sure if it's true. But most of the people here, I noticed that they go to Nationwide, and they have a lot of good stuff to say about them.
