Guys on the supplementary info-your travels, if i dont have dependant aside from my spouse do i need to write not applicable/none or i just leave it blank..

Guys on the checklist if it says copy/copies does it mean photocopy only? Not certified true copy, i just want to make sure.. Thanks

Put the photos SEPARATELY in a small envelope. Write the name of the person on the photo on each envelope. Attached the small envelopes in a blank sheet of paper. Don't forget to write the name, date photo taken, D.O.B. at the back of the 5 photos. The mailing labels you can clip in another blank sheet of paper.
In my case, I forgot to put the barcode pages on top. I presume that it should be placed on top of IMM0008.

Check original for the official receipt. Don't forget to fill-it up.

Here's the sample of what i wrote at the back of the photos: Name: Family, Given Date of birth: July 1, 1986 Date taken: May 25, 1986 i hope there's no problem with that.

Then the envelop containing the photo atached to a blank sheet should still be place on the order indidated on the checklist, same with the mailing label?

I think everything is OK! But one thing more, I hope that they can recognize you in your photo...it was taken May 25, 1986. Was it a Sonogram photo??? ;D ;D ;D

Do I need to fill 2 rows to show a different status in the USA in my case??

If no, what status should I choose from the list?

I can't remember if there is a facility for viewing the uploaded documents BUT what I do remember is that you can print a "Document Summary" and "Application Report" by clicking the appropriate buttons. Anyway, the online system will generate an error if in case your application is incomplete.

You have to upload BOTH Transcript and Diploma for secondary. If diploma is not available, upload instead a letter addressed to MPNP citing the reason why it's unavailable. Link for self-Assessment form below...

Yes include that also parekoy, as it will added to your credentials, you're an IT guy also??
