Hi guys,

I was just looking over my Supplementary Travel Info form (IMM 5562), and noticed that I have made an error and written down the wrong month for one of my travels. I should have written 12-2002 to 01-2003 (20 days) and accidentally wrote 05-2002 to 01-2003 (20 days). I had all of my travel written down on a piece of paper and was copying it from there, and accidentally read the month from a different line, and even missed it when I was proof-reading later. Just saw it today. For clarity, the travel was to my home country. The dates I wrote down are obviously not 20 days and this is a clear error.

Should I send an updated form and clarify the error? I'm really worried that a stupid mistake will cause major problems. Could senior members please let me know what I should do to avoid issues?

I do not think this is a major issue, however, you can send an email to CPP Ottawa explaining the mistake and ask them if you should submit a corrected form.
 
Thanks Sashali. Could you please comment on why you think this is not a major mistake? Instead of the 20 days I traveled, it now seems I was staying in a country other than the one I was in for 8 months!! That would completely contradict the information in my background declaration. Should I not send them a corrected form anyway?

You can re-send the revised supplementary form to ottpilot-immigration @ cic.gc.ca. Please put your application number in the email subject and give a brief description about the document.
I had a similar issue with one of my documents, I emailed the revised one to them. They responded within a week, saying that they added my revised document.

Similar to yours. Dates related in supplementary travels form :)
