Missmatch in birth date

Hi All
I need an expert advice. Please help. I am Indian citizen, now in USA on L1 visa.
I am planning to apply for canada PR. My birth date in birth certificate is different from all other documents including passport.
How to proceed? will an affidavit do? If yes, how can i get it as i am not in India now?

Any advise is welcome
