Hi Gurus,

I have a quick question. I am working in Canada and my wife is in India with kids (All are Canadian citizens). One of my kids passport is expiring next week. I have couple of questions about the application pptc042 and consent form pptc028

1. In Section 2 PPTC042 form, my wife will sign and date as an applicant and i don't have to sign and date as i am out of country ?

2. Where can i find the PPTC028 parent consent form ? Do i need to go to Toronto Passport office to sign Infront of them? Delhi embassy gave a file number for reference.

Please advise -

The other parent is living in another country. How do I get their signature for consent?

If one parent is living in another country, they do not need to sign the application form submitted to the High Commission or Consulate General. Their personal details, however, must be provided. The parent living abroad must contact the nearest Canadian passport issuing office to complete the “Acknowledgement/Consent to Provision of Passport Facilities for a Minor Child” form (PPTC 028). This form is to be completed and signed in the presence of the passport officer. Be sure to bring your passport to the issuing office. The form will then be faxed to our office.



The other parent is not able to sign the passport form. What can I do?

The parent or guardian submitting the application should provide copies of all legal documents pertaining to the custody of the child.

If it is not possible to get the signature of the other parent, you must provide details of the other parent, such as:

  • name
  • date of birth
  • address
  • contact telephone number(s)
  • last known contact
  • details of attempts to locate the other parent
Advise the passport officer when you submit the application that the other parent cannot be located

Thank you. I had filled pptc028 today at passport office to send it to the Indian embassy. I have another question, my wife will be an applicant and i will be the other parent where i don't need to sign as i am here in Canada also filled the pptc028 form for consent. How about Guarantor in this case which is section 6 in the child passport application? Since my wife is sending the application from India, she can't be the Guarantor right ? If i can be Guarantor, i can't sign ? Any advise...

A parent or legal guardian cannot act as guarantor when applying on behalf of a child or dependent adult
