Hi - I got a message in my CIC account as well as email to submit scanned and completed copy of IMM5475.

I am not sure why it is being asked since I did not opt for a designated individual ??

Was anybody else asked to do the same ??

Any information/help here would be highly appreciated.

Hi, It is like you giving authority to a representative of CIC to know the status of your profile.Like if you are doing through consultancy/agent, If you give this form only they will be notified with your status update of EE.

Upload a signed affidavit/LoE that you wish to receive all communication to yourself and you did the application by yourself.
