CHC has appointed and designated laboratories and clinics to conducts Medical test. Once you receive the request for medical, check the designated list on the website of CHC, as per your residence status, and visit. They themselves will let u know the type of tests.

chest x ray
urine testing for sugar , blood and protein
blood test for VDRL and HIV
some other tests like creatinine if you have an auto immune disease
