Medicals and Police verification requested! Need help quickly!!! - update

I am new to this forum, but find it very useful.
I have received a letter from the Canadian Consulate General, Detroit stating that I should get my medical test done and need the FBI and police certificate for India and USA for both me and my wife. I have the following questions.
1. The letter says that the medical forms are enclosed. When I checked the mail, there were no forms excepting the letter. What should I
do? I already faxed them and emailed them. Is it possible to just download the forms and take the medical test. The doctors office asks
me to contact the canadian consulate, but the consulate doesn't answer via phone.
2. My daughter is 6 months old. I already sent her application fees and updated the application since she was born after I applied in June
2007. The letter the canadian consulate sent has asked me to send an application fee for her. Is that necessary? I only pay $490 X 2 for
me and my wife for the RPRF right?
3. Does my 6month old have to take a medical test too?

I would greatly appreciate some help.

My friend is also having same problem of not receiving med forms. what did you do ? Please let me know. Do they asked you to send RPRF?

Hi,

You did the right thing. CHC will have to send the medical forms, they cannot be downloaded. You will also have to pay the fee for your child, 150 dollars I think. there is no RPRF for children, but application fee is a must. yes, the child will also have a medical, although certainly not like adults. Just a look at the child, a qucik check by the DMP, thats all. While you wait for the CHC to send you all the forms again, I strongly advise you to get your PCC and FBI clearances done. These might take some time.

Children below 11 yrs need to undergo only urineanalysis. No blood tests nor xrays.

Regarding medical forms missed out please intimate ur Visa Office at the earliest and they will surely resend again.
