i got request for medical but no forms is attach to it ? wat shld i do ?

I had the same situation for my wife. I Emailed them and they had provided the reason for not having the form. If you are primary applicant then you ahsould ask for medical form, in my wife case, i had done medical without medical forms as forms are available with dmp

kindly tell us the VO sent out the MR form on your praper mailing address or via your email address
