Take your passport and if you have any recent medical records take them. I also am American and they did not ask me for the things you mentioned.

Hello...i had a Scoliosis surgery long time back around 18 years back and still having rod in my spine will this be a hurdle in medical examination for the PR

For PR through family reunification you should not worry about any medical condition except for those that might present a risk to general health (i.e. infectious disease). The applicants of family reunification are exempt from being inadmissible on the ground of excessive demand on the health system.
