Can I complete the medical test before the ITA with the Express entry profile id or the doctor would ask for the ITA.

You can but its recommended to do it at a later stage since your deadline to land in Canada is usually based on your medical date. Medical is valid for an year and you have to land before it expires, in most of the cases.
