how did you got this? by email on on cic account? and did you do medical exam before submitting visa?
Of course it does given long term a smoker could develop lung cancer and become a big drain on the healthcare system. Many healthcare systems even use the fact someone was/is a smoker to exclude them from some medical procedures as high risk depending on their overall medical history.
Whether without any previous medical issues it affects a PR application is doubtful but is still something that should get attention in a medical through a chest xray.
