Medical surveillance 2.02 in section 28. Please help me

I got My CPR and other immigration documents. In the attached letter they discuss many topics one of these talking about Medical surveillance. I have code 2.02 in section 28 of the CPR!! what dose 2.02 code mean?? Also the office did not provide me with any separate surveillance form. Is it a problem to have this code?? please help me

Send your VO for it. They will send it. In airport they will ask for this form.
