One question: how does the visa office send the medical request? Via email or regular mail?
Can anyone please tell me the text of the letter?

Usually via postal mail. At least that's how mine came.

It should be postal mail.
They will normally give you Form A with your photos pasted on it and some basic information typed.
I have knowledge of how it works in Buffalo office (my case).
However, I have reason to believe (forms needed) that this is the usual method for all.

Hai Nen,

To some extend it depends on the visa office to which you are connected with. In CHC New Delhi, normally they sent ordinary post. But some postings in this forum mentioned that they have received e-mail also. The medical request come with Medical Form, Client id from etc along with instructions.
