Can someone send me the 3 attachments (the medical form, instructions for me, instructions for the DMP), which you had received with medial requests, removing all your personal information if any?

can you please let know what is the relevance in which you are asking for the 3 attachments ? what exactly are you trying to achieve ?

No one can send you these forms. If you applied prior to 2nd jan 2013 and intend to do Medicals in advance, contact a DMP to arrange - assuming you have a UCI.

Alternatively, if you want to see what is involved in the medical, all the forms are available on the CIC website !

Agree... and no one should send them just for privacy/security reasons alone. Even when you do your best to remove personal information, it's still possible to miss some, so why risk it? The other option is to wait(patiently) for their turn.
