Hi,
My wife is an American citizen and I will be sponsoring her outland this summer. My question is about medical history.
Twenty years ago, when she was a child, she attempted suicide due to an abusive step-parent. She was in the hospital after for about two weeks, saw a counselor for a while, and was on (what we think was probably) anti-depressant medication for about a year. Since that time (after the abuse ended and she no longer had contact with her former step-parent) she has never had any treatment, either medical or psychological, for any mental health concern. Like I said earlier, this occurred two decades ago.

On Schedule A, do we need to say yes to the question about any serious disease or physical or mental disorder? We don't know if she was actually diagnosed with anything at that time.
If yes, should we try to obtain the medical records? We have no idea if the hospital is still open or if it's closed, and it's in another state. I'm not concerned that it'll be a barrier to immigration, just that we want to avoid any possibility of being accused of misrepresentation, and we also want to avoid delays that would be caused by requests for medical records.

Right now, we have checked yes on Schedule A and offered a short explanation of what happened and the fact that she has been mentally healthy since that time. Is this correct?

A situation like that so long ago, I would not even bother to mention it in the app or the medical.

Wouldn't we risk having her banned for misrepresentation? But I guess we don't know if she was diagnosed with any mental health disorder. I just don't want to mess up our app, or delay it if they request records.

They aren't going to know unless she tells them or she has physical evidence of the attempt that will be seen during the medical. IRCC cannot access her childhood medical records.

It is not a reason for refusal if you want to tell them.
