Medical records for proof of RO

Where I can get a print of my medical consultation history in Canada, to submit as a proof of residency. I checked with my family doctor and he said the Service Ontario will provide one up on request. When I called Service Ontario office, the Agent said she is not aware of such thing.

Any suggestions please?.
