It means "take the letter to the Panel Physician to do the required tests".
Maybe they have had theirs extended?
Yes, you will have to pay whatever fees are required by the Panel Physician to redo the medicals.

X-ray and physical checks, no blood and urine tests.

I received a similar email today except that I have applied under PNP and they have asked to submit all info within 7 days.

In the email they have asked to send Duly completed Re-assessment Form (attached), but there is no attachment in the email. Wondering if you were in the same situation and how did you proceed.

Appreciate your response.

can you plzz tell me are these are adtional test or conseder remedical? we got samething july 27 2014 and est done on augest 26 2014.bce our medical has expired on july 30 2014.i order gcms note its says medical expired

I think it depends where you do the tests... I have received a same request and panel physician in USA said that this request means physical checks exam which includes chest X-ray, urine and blood test. So in other words it is a complete medical again just like we did first time.

Hi , I'am from Pakistan as well. I got the same email today. Its weird to receive the email with subject Delhi in it.My first thought was may be the immigration officer made a mistake . How can they ask you to send your medical record to India ???

I ended up doing chest x-ray and just physical examination (No blood/urine test) and I went through smoothly. Although I believe only chest x-rays were required, but medical staff was not aware of the exact test that is required so we just did the physicals to be on safe side.
