Medical Procedure and length

As far as documents to take with for the medical. Is it just Appendix C.. it is just one page with some basic contact details..

Yeah, but double check with the doctor, because sometimes they ask for things like prescription lists, etc. As far as immigration goes all you need is appendix C and the photos.

I am processing my application outside of canada through london, uk. On the section C sheet it asks for the visa office that will be processing the file.. does this mean I write UK office or is due I use the toronto office.

You'll use the London office. The Mississauga office processes the sponsor's information, not yours.
