Where/how can I get the referral form the embassy? Do I need to go there directly or can I just download it from the website?

just bring the appendix c form,photos,and passport.sa nationwide health systems ako,punta ka lang dun madali lang naman pero it depends kung maraming nagpapa medical

ako po walang dalang form, dala ko lang ay MR, pictures, passport, pero para sure ka dalahin mo na din ang appendix c form. BTW sa st lukes clinic ako nag pamedical.

last january 10 po, sabi ng staff nila 7-10days ang processing ng medical nila tapus sila ang magpapasa ng result sa VO. kapag merong problem tatawagan ka daw nila. so far wala naman tumawag sa akin since then, i checked in ECAS my status is "MEDICAL HAS BEEN RECEIVED", after po noon wala pang ibang update. medyo nakakainip, pero ok lang kasi dito pa naman ako sa abroad eh.

Thanks for the reply. Clarification tho, it says in the region specific guide that we need that IMM 1017 form? Will Nationwide be the one to provide this?

And wait, so it it advisable to have the check up in St. Luke's instead of Nationwide since I read in one of the posts that they're slow in submitting the results?

Also, once Im done with the medical when do I send my application to Mississauga?

ok naman sa nationwide system very accomodating ung staff and mabikis lang din pamedical... 7-10 working days din depende kung may prob sa medical mo tatawagan ka

Did you submit two copies of pictures? Since its needed under proof of relationship (region specific requirements) and in one Of the forms aswell (wedding pix, reception, pics old n new)

yup not only two but alot of pictures but not all as long as it can show u the passage of time like for example wen u started ur relationship up to the recent photos u have that u are together
