Medical issue for non accompanying child

Hi
I have work permit and all my PR process done. But CIC want medical for my daughter who is 16 years old and live with my ex-husband husband since 8 years after our divorce and he has her full custody.
I am unable to find her and her father for medical . He delete his Facebook account and change his phone no. ,Also move from last address.
Please suggest what documents required to confirm this?
I already put custody papers and affidavit about not to sponsor her in future.

What we did with my husband's then 17 year old was write a statement saying that "Joe" was 17 years old and was not going to be coming to Canada and that he refused to do the medical and we understood that we could never sponsor him in future.

Of course you can't say that your daughter refused to do the medical, but you can write a statement saying that you are unable to contact her to do a medical and that she is not your dependent child. You also have to acknowledge that you can never sponsor her in future. You can also include something about how you have tried to contact your ex husband to get a medical but he's nowhere to be found.

Provide the same documents again, highlighting the custody agreement showing you have no custody, along with a letter detailing efforts to contact them and why you were unable to.
