I will be doing my medical for my outland common law sponsorship application this week. I will have my application processed in Warsaw.

My question is what information should be given to the medical location? I believe it is the following, so please correct me if I'm wrong...

1. I am doing the medical for an "Outside Canada" PR application
2. The Visa Office responsible for my application is Warsaw Poland
3. My UCI from a recently failed CEC application? (I assume I can still use this number for this new application?)
4. I pay the fee and provide one photo per the specs (as told to me by the physician's office)

From the panel physician I will obtain Copy 1 of the Medical Report - Client Biodata and Summary (IMM 1017).
 
Actually, I think I figured out that I can reuse the UCI from my previously failed CEC application. The UCI simply follows you everywhere and is the exact same number CIC gives you when you do a PR application as was written on your study permit or work permit! So, definitely will give it to the panel physician!! ;D

I think to get the specific answers you need, you will need to contact your panel physicians office. All I needed to supply was my passport and let to them know that I was doing an upfront medical for family class. The office staff took photos, I also already had UCI from previous work permits, but they didn't use it. I didn't even need to let them know which VO as it is all electronic.

It sounds like from your post you will need to supply a photo, so different from my situation. Best to contact office to get specifics for you.
