It will depend on whether your chosen Canadian immigration panel Doctor is using the CIC eMedical system or not.
If they are, then it is next day.
If not, then you have to cope with the vagaries of both the postal system from where the Doctor is based, then CIC processing times once they receive it, so likely to be a couple or a few weeks from South America I would guess!
If you have a choice, ask them if they are on eMedical or not and go with the one that is!
