Just realized the forms for inbound/outbound medical are different. My wife is in Canada but we're leaning toward an outbound application. Does it matter which form we take to the Dr.
I had the same problem.
When I went to the Immigration Doctor (In Canada) he gave me the original medical form to send with my application (Out land application)
