medical form for kids under the 16th year of age

Can someone that has done that or knows about it, should we send this form with the rest of medical exams or just have it with us when we go to Canada? The paragraph on this form is confusing as it speaks about it being important to children while registering for school in Canada.

Any thoughts?

What form? Their vaccination record? If it is that, then if the school doesn't have a copy, and there is an outbreak of measles mumps etc. they will be sent home.

There is no school yet! We just want to know if we should send it with the rest of the papers they asked for or keep it and take it with us to canada!
