I am applying to sponsor my husband from ukraine. We are applying for sponsorship outside of Canada. Ive got pretty much everything we need including translated and originals of police clearances. I am confused on whether he is suppose to do this medical exam BEFORE I send the application or will CIC request the exam when they begin reviewing the PR part of the application? I would appreciate any information!

Do it before, and as close as possible to the date you actually mail in the application.

If you don't do it upfront, CIC may claim you submitted an incomplete app and return the whole thing to you. Also by doing it upfront, it prevents delays in processing later since they won't need to ask you to do one and stop your application while waiting for the result.

And does my husband take that form thats listed for the medical examination back from the doctor and send it to me so I can include the original of it?

sometime in mid march :)
The Dr should automatically send it to CIC and give u a piece of paper that u will need to send the original that they give u. When my husband did it it was a piece of paper to prove he had it done and CIC was able to pull it offline the results.send the original of whatever the Dr gives u and make photocopy for u. Also it has to be a Dr that Canada has listed for that country. So yes they have it on their website :)

Yes, here's the link to find a local CIC panel physician: http://www.cic.gc.ca/pp-md/pp-list.aspx

And yes, the doctor will give you a receipt that serves as proof of the medical, and that needs to be included with the application.

Another question regarding this..

Is there a form he takes with him appendix c? Does the doctor have specific forms they mail. I cannot seem to find a printable version of the medical exam form.

There is no medical exam form for the upfront medicals. Make sure when you husband calls for an appointment he tells them he is doing an upfront medical for family class sponsorship. The doctors now use e-medicals, and they submit everything to CIC electronically, and the doctor will provide your husband with a receipt . Send the original of the receipt with the application to CIC and they will match the e-medicals your husband did with your file (don't forget to make a copy of the receipt for yourself).

There is no form, but tell him to remember to take his passport for identification, the payment the physician requires, and any medical records he can get from his own GP. He may not need photographs; many doctors now prefer to take their own with a digital camera because it is easier to incorporate the photos in their report. Inquire of the DMP beforehand. For a full list google CIC medical examinations.
