Medical Examination Instructions
Instructions on how to take the medical examination will normally be sent to you after you submit your application to the visa office.

But the medical exam is supposed to be sent with the application, so why would CIC want to wait until after we send the application before giving us the instructions on how to do it?

I think the sentence you quoted is only meant for Federal Skilled Worker applicants. For sponsored people, they need to do the medical exam before sending the application...at the end of the medical exam session, the doctor will give a piece of paper that you're supposed to include with your application, and that piece of paper will be the proof that the principal applicant did the medical exam :)
