Medical Exam Form for Ukraine

Hello!

I am sponsoring my wife from Ukraine, I have read all the medical instructions.

She called the "Panel Physician" in Kyev Ukraine to get her medical exam done. The panel physician told her that she should bring some sort of a form that can be obtained in the Canadian Embassy.

What in the world is he talking about? I have read on the website that, he is supposed to have the forms since he is one of the CIC appointed doctors "panel physicians".

I do not know what form he is talking about and we were not able to reach the Canadian Embassy in Kyev, they keep putting us through a bunch of automated questions and then say "Look on our website".

Any suggestions?

The panel physician's office should either use e-Medical (online medical report system) or have IMM1017B/E in stock at their office.

AFAIK, it's not available to download online.

Thank you. The issue has been resolved. There is no need for a form. The panel physician provides examination form.
