My wife made a doctor's appointment with a panel physician for her upfront medical. She was told to bring her passport, photos, and the immigration form to fill out. I assume they mean the IMM 1017, but I thought this wasn't done anymore. Do I need to bring a form? Which form? How do I get it?

HELP???????
 
you are doing an upfront medical and do not need a form. you are correct in assuming that form is generally not used anymore. the doctor should be familiar with the emedical system and know that all the information gets entered into the computer, from which the doctor then prints out a receipt for you to include in your package. i would suggest making sure the doctor's office is clear about this and knows how to use the system, and if not report them to cic (and tell them you will be) and find another doctor on the panel list to go to.
