Medical Exam for family members???

I just read now on Express Entry website that in order to apply for PR I need to take a medical exam which is fine, but they say that my family members must also take an exam even if they are not coming with me !??

Which family members? I have no spouse or kids, I have 2 parents and 2 siblings. Can anyone please give me some more information on this?
