Medical Exam: BEFORE or AFTER submitting the application?

Hi everyone,
I am a bit confused about WHEN the medical exam has to be taken.
Me and my common-law partner received the ITA under the EE program. We are applying outland, from our home country.

We have the page with the checklist of documents to provide. I see:
Proof of medical exam (required)
The help text reads: "You require a medical exam. Learn how to get an upfront medical exam. In order to submit your online application, you will need to upload the information printout sheet or the IMM 1017B Upfront Medical Report form. Your doctor will give you one of these forms when you complete your medical exam."

But, on the related help page on CIC I see also the option: Wait for instructions once you have submitted your application
"We’ll send you instructions on how to get your medical exam done. You must go for your medical exam within 30 days of receiving these instructions."

So, which one is correct?

You won't get any instructions, book your medicals and submit them along with your eAPR.

I am confused too..
I just called up the local Panel doctor on Friday, they said they were recently told by the Canadian Consulate General here that they do not do upfront medicals anymore.... explaining that there were some recent updates.
They only do Upfront medical checks for "workers" now...

Tell them specifically that you are an EE candidate and need to do upfront medicals. At the most, they will ask you for a copy of your ITA

While sponsoring parents, do we need to have a medical exam before or after submission of application?
