medical exam before mailing the application

Hello! Merry Christmas and a Happy New yesr for those who celebrate)))

Could someone remind me how to go about medical exam before mailing out the actual application (spousal sponsorship)?

I thought there was a special box to fill out - client ID, file number something like that...
but if application is not in progress yet, we wouldn't have any id's or number to put there.

Also, I thought the applicant was responsible for digging out that form IMM1017 and bringing it to the panel physician person, but apparently that form is not available on the cic site.

If anybody knows anything, please drop a line or two

It's in the country specific forms on the CIC website. Make an appointment with a designated doctor and take in the form and however many photos they ask you to bring.

The CIC changed some of their medical policies a few months ago, so apparently the IMM1017 form isn't required now by you, and the doctors are supposed to have all necessary forms in their office. However i would still call the doctors first to make sure they are using the same info so there are no surprises when you get there.
