You fill out Appendix C of the Regional Specific Guide (they are all the same) and give it to the doctor. They will provide you with a receipt that has your picture on it and some information (name, address, etc.). That's the receipt you include in your application.lomeli619 said:so since im from the states i take the american form and they can fill it out? so after that i will be submiting my forms 2 weeks after that do they give u some kind of voucher or the results or what
They need to be immigration sized photos, and u will need more than one. Each doctor requires a different amount so make sure u find out how many u will need. For my husband's medical in Dominican Republic, he required 5 immigration photos for the medical.lomeli619 said:ok lol so i take the pics and take one with me with the form, this whole process just seems super confusing 2 me haha thanks tho guys its like the passport pics right
Yes. Ours required 8 photos.lomeli619 said:wow that many ? so it would be best to call and ask ?
