medical certificate is valid if applying from another country??

As per requirements, Pakistani residence needs to examine medical tests. If my wife get medical test report from Karachi Pakistan and apply from Saudi Arabia Riyadh embassy, will it be considered OR she need to go for re test in Saudi Arabia. In simple words my question is "Is medical certificate obtianed from Pakistan is valid to apply from anywhere in the world OR you need separate medical test if u are applying from outside Pakistan?

What if i applied from malaysia for my visa (online)
And i got approved while im in saudi? Do i need to send my passport to malaysia or i can do it in saudi?
