Medical Booking Question

Hi, I am about to get a date for medical for Express Entry and i need the details for the following points they asked.
4. Visa type
8. Medical Letter from CIC /Upfront Medical/IFHP letter. (no need of letter)

What will be the visa type and for Express Entry what would be in the 8th Point.

If you are going to one of the approved medical centres by IRCC i dont think they would ask you these questions. Check the link below
