Hi...
just confused about the medical exam for outside application, i jut called the panel physician on the official website of canadian immigration, the representative told me that i have to download the appendix c and as i read about the updated process on the medical exam its said na,no need to download the forms as it could be provided by the panel doctor but the representative insist i have to download the appendix c, in my mind i wonder how to download if its not already on the website about that downloadable form...

i need help on this anyone who just had thier exam in manila.... i need advise on it.. thank u

eca, same issue din ako dyan pero eto un site about appendix C form http://www.cic.gc.ca/english/resources/publications/dmp-handbook/pdf/AppendixC_e.pdf
explanation nang iba eh most of the staff didnt know the new rules kasi the old ones doin that for long so nakasanayan na nila siguro. but to make sure as they requested it mag provide nalang tayo nang Appendix C form make a copy na madami na at sagutan then isama sa requirements. baka sa St lukes din ako mag pa medical but still waiting na gumaling muna ako from colf and flu. hope this help you a lot...

i c, thanks sa link, i called back st.lukes,at buti nmn at rep na may knowledge na bout the new system ung kasagot, walk in lng pla dun first come first serve.. san ka bound??? ready na ung application forms and etc mu??

anu sabi nang Staff? can u give me info. naku di pa tapos lahat nang requirements eto nga nalaman ko nanaman need ko nang advisory of marriage (form5) from NSO. medical, some forms need tapusin, yun police clearance ko sa dubai di ko alam if need pa kuha ulit kasi paso na, mga questioner at mga proof nalang na kasal kami. so totally madami pa pala. ikaw po? kailan din u papa medical?

what do u mean about the advisory of marriage,kasal ba kau?? san?? kc kung kasal kau marriage certificate lng nmn kailangan, almost done na ako,sa medical exam nlng ako at pg organize ng mga document at proof...

opo kasal po ako sa sponsor ko: cenomar pang single AOM pag kasal na kayo. i just got this information today lang din so need ko balik NSO to get this form

eto po yun sa kit from pinas to canada req:

1) For the Philippines
The following original documents are required for you (principal applicant, regardless of age) and
for each of your dependent children, if applicable:
• Birth Certificate
• Marriage Certificate
• Death Certificate
• No record of marriage (CENOMAR) or Advisory on marriages
These documents must be issued by the National Statistics Office (NSO) and printed on NSO paper.

kc sa pag kaka alam ko, CENOMAR ung tinutokoy if ur not married to ur sponsor pero pg married kau, automatic na marriage contract galing sa NSo ung kailangan, eh as far as i know even if ul asked NSO for CENOMAR but then if ur married it will appear the marriage contract that u had with ur sponsor.. so i guess no need to provide the CENOMAR since ur legally married , i guess which ever applies to u are the only document u need to submit, kc pwede din CENOMAR ung kailangan for those common law relationship meaning not wed.... ...
