we have received our medical request email after quite a long wait time due to correspondence we never received and which did not show up in the status update. We finally got the email listing all family members with UCI numbers, but the attachments i.e. the IMM1017E forms were missing. Is it critical to have them for the doctor's appointment, or is the UCI sufficient - actually it was for one of our family members who did the exam in Germany, but we are in Canada and who knows how it is here???
