I booked E-Med exam for Tomorrow. I received an email From cpp-o containting as attachement IMM 1017 with medical instructions and one page MEDICAL REPORT CLIENT BIODATA AND SUMMARY, however when I booked the appointment I was asked to bring a personalized form from cic with a barcode on it. (Which I didn't receive) According to the receptionist I spoke to, for the e-medicals they need to scan a bar code. Is this true?

Those who received med request, do you have a barcode on top of your document received? Can I proceed with my medicals without that form?

Is this what everybody else received in pdf attachment to the medical request email? ???

Medical Examination Instructions
Medical instructions are issued when an application has been processed to the stage where the required
medical examination(s) should be undertaken. Please read the following instructions carefully.
The medical examination must be performed by a physician from the CIC list of panel physicians. This
list is available by accessing the following website: http://www.cic.gc.ca/dmp-md/medical.aspx.
You should identify a panel physician in your area and book an appointment for your medical examination. When you schedule your appointment, please ensure that arrangements are made for your chest x-ray and laboratory tests as well.
Medical examination results are valid for a twelve month period from the date of the examination.
All costs related to the medical examination, if applicable, are your responsibility and are payable at the time of the examination. Should your application be refused following the completion of your medical examination the costs will not be refunded.
Please bring the following to your appointment:
- The attached medical report form
- Proper identification (proof of identity must include at least one document with photograph and signature such as a passport, driver's licence or national identity card)
- Eye glasses or contact lenses, if worn
- Any medical reports or test results that you may have as a result of previous or existing medical conditions
- Four recent photographs, if the panel physician is not eMedical enabled (note: panel physicians that are eMedical enabled are indicated on the CIC website listed above)
Note: It is important that you inform the panel physician of any previous or existing medical conditions. Failure to do so will delay the processing of your medical examination.

Hi, haven't gotten the MR yet, but from what I've heard, sometimes you have to look for an attachment to the email. It can be an .html attachment that you have to view. Perhaps there you will find the image of the barcode.
make sure your email client doesn't hide anything, and look if there are any attachments to the email.

Does the form have your personal information already filled in? Like your name, address, application # and an IME #? Or did you get a blank form/template? If it is the latter, then you'll have to contact CIC.

The first thing that I was asked when booking the medical was the IME #; the person on the phone went on the computer to pull up my information from the eMedical database using that IME #. So don't think you'll be able to do the medical if CIC has not provided you with an IME # in that form.

I got the exact same form that you did by the way, but all my personal information was already typed-in and there is a bar code in there too.

My personal information is not typed in the form just empty boxes and there is no IME number. I emailed CPP-O....
Anybody faced the same issue before?

Yes, MR forms have personal information and barcode. You need to update your acrobats reader software to the latest version and you will see all those information on the attached PDF form!

Cos, thank you so much for your response, I did in fact open the attachment on a different computer and you're right, all the information is there, and the barcode. :) tomorrow morning I'm going for my appointment!

I got the IMM 1017 E form which I took to DMP and got done with the medical exam. The nurse at the office did not know if IMM 1017 B form was also needed to be filled?

My medical form too did not have any barcode and I wasn't asked for it by my DMP.

Sorry guys my form too had a barcode :) , Didn't remember about it when I printed them. When I look at the form on my phone, it shows blank :D
