It's just a normal part of the process. Once they've approved the sponsor and requested the biometrics and medical, they transfer you to a visa office. Once it's there, they'll review the evidence you submitted to prove your relationship is genuine. You might get a request for additional documents if they're unsure of anything, but otherwise the next thing you will hear is the request for passport if they approve everything

She can give her medical in Canada if she goes to an approved panel physician, but you can't give biometrics for PR applications while you are in Canada. You have to go to a VAC, and there aren't any of those in Canada.

I believe multiple people have already told you that it's also perfectly normal for applications to get transferred without having received a medical request. I don't know how they decide who will get the request at the VO, or if it even means anything, but it happens. As has been said several times, there really is no official order these things happen in, it varies case by case. It's nothing to worry about!
