Hello,

My husband will apply as the principal applicant. After the marriage I took my husband's surname.

My question is: does my marriage surname is considered as my "Family name" (as stated at the "Generic form" and "Additional dependant form"), and I should write my birth name under the "Have you ever used any other name?" question, or is it vice-versa????

Yes, it is your family name (surname) now. You write your maiden name "used another name"
